Request for Proposal Emergency Preparedness Plan 

Peterborough Family Health Team (PFHT) invites submissions for a Request for Proposal for an Emergency Preparedness Plan. 

The proposed exhibit will support the mission, vision, goals, and objectives of the 2016 – 2019 Peterborough Family Health Team Strategic Report. Please provide four (4) copies of your proposal, one (1) unbound signed and clearly marked as ORIGINAL and three (3) copies of the original proposal clearly marked as COPY and one (1) electronic copy (Microsoft Word or Adobe Acrobat PDF) on a flash drive in a sealed package or envelope. The original and all copies should be identical. 

Due Date: The proposal is to be delivered and/or couriered at your own expense to the address listed below by Wednesday, March 8, 2019 by 1:00 pm, local time.
